911 Emergency

Envelope

1. Write 911 on an envelope in big letters
2. Indicate which arm you need your BP cuff used on, and circle Y or N for Intubate.
3. Include the following 5 pieces of information in the envelope:

  • DNR (Do Not Resuscitate)
  • Spreadsheet 7: Important Phone Numbers
  • Spreadsheet 9: Prescriptions
  • Medical Directive
  • Copies of Medical Insurance cards (front and back)

4. Put copies of 911 envelope where you can grab it and go:

  • Back of front door
  • On your refrigerator
  • Glove compartment in your car
