911 Emergency
1. Write 911 on an envelope in big letters
2. Indicate which arm you need your BP cuff used on, and circle Y or N for Intubate.
3. Include the following 5 pieces of information in the envelope:
- DNR (Do Not Resuscitate)
- Spreadsheet 7: Important Phone Numbers
- Spreadsheet 9: Prescriptions
- Medical Directive
- Copies of Medical Insurance cards (front and back)
4. Put copies of 911 envelope where you can grab it and go:
- Back of front door
- On your refrigerator
- Glove compartment in your car
